Media Release Form
I hereby assign to APO Nutrition and Health Center the right to use my likeness, voice, and name on promotional materials and programs presented in print, on the APO´s Web site, and radio and television broadcasts. I also hereby authorize the editing, reproduction copyright, exhibition, broadcast, distribution, and promotion of said material and programs by APO Nutrition and Health Center without limitation.
I understand that I won’t receive payment for participation in these material and programs, and I hereby release APO Nutrition and Health Center from any claims for remuneration for any subsequent use of these materials and programs.
I indemnify and hold harmless APO Nutrition and Health Center against any and all claims arising out of any actions or statements I make or program material that I provide, including, but not limited to, any claims in the nature of libel, slander, copyright infringement, invasion of privacy or publicity right, and errors of omission. I expressly release APO Nutrition and Health Center from any privacy, defamation, or other claims I may have arising out of broadcast, exhibition, publication, or promotion of these materials and programs.
